QI project triggered by falling KPI and a negative patient experience ‘I wish I could have gone back home’. The aim is to improve KPI 6: return to original residence to the national average (70%) by 1/06/2022. This is due to our KPI 6 falling below the national average October 2021: 68% in comparison to national average (70%) with further downward trajectory Pre COVID: 79% of patients returned to their original residence.
